What is it for

Applicants for driver’s licenses, student permits, non-professional licenses, or professional licenses in the Philippines must fill out the ADL Form. It is used by the LTO for registering motor vehicles and obtaining Certificates of Registration and Official Receipts. The form is also used for the following transactions:

How to Use the ADL Form

It is pretty straightforward to fill out the ADL form. It has slots for filling out personal information needed to apply for a license. If you make a mistake in the information provided on this form, your license will have inaccurate information, which will be quite time-consuming and expensive to rectify.

ADL forms are free of charge. Whether you secure it at the LTO branch of your choice or download it from the LTO website, the form is free.

How to Get it

Visiting the LTO Office

In person, you may secure the ADL form by following these steps:

Step 1: Make sure all the other documentary requirements for the LTO office transaction are prepared.

Step 2: Schedule an online appointment for the LTO once the requirements are complete.

Step 3: Visit your preferred LTO office on the day of your appointment.

Step 4: At the service counter, state your purpose and obtain the ADL form.

Step 5: Complete the ADL form and wait for your turn to submit the required documents.

Downloading from the LTO website

Follow these steps to secure the ADL form online, via the LTO website:

Step 1: Visit the LTO website (www lto.gov.ph).

Step 2: Click Menu.

Step 3: Select LTO Forms.

Step 4: Find the Application for Driver’s Licenses by scrolling down.

Step 5: Click Download.

Step 6: Save the .pdf file to a location of your choice.

Step 7: Delete the original file and rename it the way you prefer.

Important Reminders

Here are some important reminders to keep in mind:

  • The ADL form must be downloaded from the LTO website with a stable internet connection.

  • It is not important whether you download the form and print it yourself or secure it onsite at the LTO office as long as you complete it.

  • When you download the form, you can complete it beforehand and bring it to the LTO office.
