LTO Student Permit Requirements

1. Certificate of Medical Fitness

The LTO must accredit this. These clinics are usually located near LTO District Offices.

2. The application form

Download the form from the LTO online website at LTMS Portal LTO, print it, and fill it out. This is also available at any LTO office. You can save time and money by downloading and printing the form.

3. PSA Birth Certificate

A photocopy and the original are required. In the event that you don’t have this certificate, you can use a Birth Certificate from your local registry.

6. Consent of the parent or guardian

Ages 18 and under are required to complete this course. In addition to the notarized consent form, parents/guardians must also submit a photocopy of their valid identification that has their signature on the side.

7. TIN Number

The process of obtaining a student permit from the LTO

1. Fill out the Application Form

Facts about Student Permit
