Renewal Application Form for Renewal of Accreditation as Driving School Instructor

It is used by employees of LTO-accredited driving schools to renew their application for or to continue practicing as LTO-accredited Theoretical or Practical Driving Course Instructors.

Application Form for New Accreditation as Driving School Instructor

A driving school instructor is authorized to practice as a Theoretical Driving Course Instructor or Practical Driving Course Instructor with the help of this form.

Inspection Report

The LTO form is used by individuals who own private educational institutions or their representatives to renew their LTO accreditation or their authority to conduct theoretical and practical driving courses.

Application Form For DS IT Provider

The Driving School IT Providers submit this form to apply for authorization from LTO to provide information technology-based solutions or services to Accredited Driving Schools for a period of five (5) years and renewable for a period of five (5) years.

Application for Student Driver’s Permit / Driver’s License / Conductor’s License (ADL)

In the past, the Application for Permits and Licenses (APL) form was used by those who wished to apply for the following permits and licenses:

  • A student’s permit

This is the authority issued to Filipinos who wish to learn driving or who are applying for their first or second driver’s license.

  • Driver’s license for non-professionals (NPDL)

NPDLs give qualified persons the right to operate motor vehicles under new DL Codes A, A1, B, B1, and B2.

  • Driver’s license for professionals (PDL)

The PDL grants any qualified person the privilege to operate motor vehicles for money-making purposes under the new DL Codes C, D, BE and DE.

  • A Conductor’s License (CL) is required

As a conductor, you must have a conductor’s license in order to work or perform your responsibilities.

  • Renewal of licenses

The process of extending the privilege given to holders of Driver’s and Conductor’s Licenses without theoretical examination may be initiated using the same form.

  • Transactions related to other driver’s licenses

There are other Driver’s License transactions that require the use of the ADL form, such as reactivating a Driver’s License, updating a Conductor’s or Driver’s License holder’s details, and requesting duplicate Student Permits, Conductor’s Licenses, and Driver’s Licenses.

Motor Vehicle Inspection Report Form

Owners of motor vehicles can request that roadworthiness tests be conducted using this form.

Application for New Authorization of PETC IT Service Provider

The LTO application form is intended for new authorizations of Private Emission Testing Centers (PETC) IT Service Providers. PetC IT Provider applicants are evaluated based on their technical ability to serve PETCs and implement interconnection with the LTO-IT System, their financial capacity to run and support nationwide operations, and whether all necessary business and legal requirements have been met.
