Procedures
To make a request for a Driver’s License Certification and Transaction History from the LTO, you may simply follow these steps:
Step 1. You can visit any LTO office near you, such as a Licensing Center, District Office, Extension Office, or License Section in the Central Office or Regional Office.
Step 2. If they process DL Certifications, find out whether an appointment is required or if walk-ins are accepted.
It is recommended that you make an appointment with LTO ahead of time if they require it.
Step 3. Obtain a Request Form for the application of license certification from the LTO office

Step 4. You will be called once you have completed the form and obtained a queue ticket from the service counter.
Step 5. Please submit your completed request form along with all requirements, including your queue number.
Step 6. Your documents will be verified for authenticity and completeness in the LTO database, and a Pay Order Slip will be prepared (POS).
Step 7. Upon receiving a Pay Order Slip (POS), you will need to proceed to the Cashier to pay the certification fee.
Step 8. Following payment, you will be issued a claim stub as well as an Official Receipt (OR).
Step 9. Present your claim stub and OR to the Releasing Center.
Step 10. Once the DL Certification and OR are handed to you, sign the duplicate copy.
